Chronic & Principal Care Management
Spokane Urology partners with Cosán, a Medicare-supported care management organization that provides monthly telephone check-ins and care coordination to help patients manage chronic conditions and achieve their health goals.
Eligibility
If you have one or more chronic conditions, you may be eligible to participate in Cosán’s care management services. Cosán partners with providers across the U.S. to help Medicare beneficiaries improve health outcomes and quality of life.
Program Benefits
If you are eligible, you may receive:
- At least 20 minutes each month of care management services
- Monthly phone check-ins to help you stay on track and receive assistance
- Coordination of care between your pharmacy, specialists, testing centers, hospitals, and more
- 24/7 access to your care plan through the Cosán patient app
- Support in setting and achieving your personal health goals
- Personalized assistance from a dedicated healthcare professional who will help you stay compliant with your care plan
